TAP® APPLIANCE – Policies And Procedures

Opening your account

You will automatically be assigned an account number with your first order. Just send in your patients models in yellow stone with a minimal base along with a prescription form, and we will take care of the rest.

Payment

We will be happy to accept payment on a net 30 basis. A service charge will be applied on overdue accounts. We would be happy to accept Visa, Mastercard or American Express credit card payments by telephone. On request, you will be provided with laboratory prescription pads from Comfort Acrylics, Inc.

Shipping Policies

It is essential that models are carefully wrapped separately and sufficient packing included in the box to preclude movement of the casts during shipping. 8 working days at the laboratory are required for appliance fabrication. If there is a need to expedite the fabrication, call us. Cases will be returned to the dentist by 1 or 2 day delivery. It is therefore appropriate to schedule the patient delivery appointment at least 6 working days after receipt at the laboratory.

Emergency Services

There are times when you will need the fastest service possible. Your patient may have severe sleep apnea or may just be going on vacation for a month. Whatever the case, you may need expedited service. If so, please call. Mark your box “RUSH”. We will expedite the case fabrication on the same night or next morning, depending on the time of day it is received. The TAP® will be returned to you by overnight next day service. There will be an expedited fee of $40.00.

Case Remakes

We offer a hassle-free 90 day remake policy. If the TAP® breaks or is somehow defective, send us a set of new models, a lab prescription indicating remake and what was wrong with the previous TAP®, the defective TAP® and the date the original TAP® was delivered to you. We will remake the TAP® at no cost and return it to you. The only exception to this policy is when we are told to proceed with a set of models that we think may be distorted or damaged. After the 90-day period and up to one year, any remakes will be done for the regular fee.

Guarantee

If for any reason the patient cannot wear the TAP® or returns the appliance to you within the first three months, return the TAP® to us. You will be given credit on your account for the full amount (less shipping) which may be applied to the next TAP® you order.
